In planning for the upcoming school year, please contact the nurse if your child has had any medical changes, will need to take medication at school, has any life threatening health issues such as peanut allergy, bee sting allergy, diabetes, seizure disorder, or will need nursing care for any medical issues.
Important Updates
No over the counter medications can be given to your student without a current signed Health Questionnaire on file at school.
Students entering Kindergarten need the following immunizations:
5 - Dtap/Tdap 4 - IPV/OPV 3 - Hep B 2 - MMR 2 - Varicella
